O servidor não está conseguindo acessar a database, tem certeza que o nome é "teste"? Como está no config.

Assim como Snowsz disse!!! seu servidor não esta conseguindo localizar o banco de dados. verifique se sua senha esta correta ou se realmente criou o banco de dados com este nome!!!
