Fala galera, beleza? Bom estou tendo um problema em meu highscores, ele ta dando ERROR 404 NOT FOUND - quando tento acessar a página. 

 

Desconfio que seja problema relacionado a Database, pois tentei fazer um reset nela e agora página está tendo problemas. Veja o código:
